| Description: | Duties and Responsibilities1 Advise medical staff on the prescribing of therapeutic diets and nursing staff on the nutritional management and administration of dietary requirements.2 Visits patients at home and develops a Plan of Care.3 Perform an evaluation / assessment of each patient’s unique dietary needs during the first visit.4 Following referral from a medical practitioner, to interview and instruct patients and their relatives/caregivers on the management of dietary treatment at home.5 Interview patients or caregivers as required to assess their nutritional intake/status and advise on specific dietary requirements.6 Assist in research projects and relevant education of medical, nursing, and catering staff.7 Assist with the production of teaching aids and diet sheets required by the Department.8 Liase with Catering Department.9 Participate in Environmental Safety Rounds.10 Responsible for keeping updated in Infection Control, Fire and Safety and Disaster Training on a yearly basis.11 Participate in the Home Health Care quality improvement activities.12 Promote health education to patient and their family.13 Participate in CQI & PS Activities. |
